Scheduling your first appointment can be a slightly complicated if you are unfamiliar with your insurance policy, with a referring court order or if you share joint custody with your child’s other parent. This page is to assist you in the process.

Your 45-minute intake appointment time does not allow much time for paperwork. The more documentation you can complete prior to the appointment, the more time you will be able to discuss your concerns.

Collaborative Divorce

Most divorces using the collaborative law process will be initiated through the parties’ attorneys. In the event a couple has made the wise decision to settle their divorce using the collaborative method, Dr. Deters may be invited to the process as one of three roles: a team facilitator, an adult coach, or a child specialist.

Equestrian-Facilitated Psychotherapy (EFP)

Equestrian-Facilitated Psychotherapy (EFP) is an experiential form of therapy involving an interactive process with a licensed mental health professional and a partnering horse. EFP is contraindicated in some cases and, therefore, interested patents will first attend general psychotherapy intake and therapy sessions prior to involvement in equine activities.

For each session, Dr. Deters is accompanied by a horse handler. In addition, barn fees and expenses are incurred above and beyond a patent’s cost for psychotherapy. A fee schedule will be discussed with you at the office intake session.

Court Ordered to Seek Treatment

If the patient(s) are referred for court-ordered treatment:

  1. Prior to making an appointment, send a signed copy of the order or mediated agreement to Dr. Deters at (fax) 859.331.2562 or (email) jeandeters@courtpsych.com.
  2. Contact the office and provide your contact information including name; address; telephone number and email address. Provide the name of your attorney and, when involved, the name of your child’s attorney. Provide the date of any upcoming hearings for which the evaluation will be needed.
  3. Discuss the fee arrangement for your court-ordered treatment prior to initiation of services. When more than one party is involved, a clear division of costs should be made (whether it be split 50/50 or along some other percentage based on court order). Dr. Deters does not provide payment plans.
  4. Allow Dr. Deters’ office to coordinate appointments and provide you the necessary intake packet. Court-ordered treatment is typically customized and requires a specifc method of intake and documentation. Very few court-ordered treatments are covered by insurance; however, some are. For this reason, a copy of your court order and contact with the office is necessary if you have questions that need answers.
  5. Complete the intake packet prior to your appointment. Appointments do not allow for time to complete paperwork; hence, if you wait until then to complete the intake packet, you will spend less time discussing your concerns and may need another appointment for such. The forms, office policies and other documentation provided to you have been suggested by the American Psychological Association and the Board of Psychology/state regulations. The volume of documentation is large, so reserve time before your appointment to complete it.
  6. Arrive with payment which will be accepted in the form of check, credit card or cash.
  7. Please remain in the waiting area and you will be greeted by Dr. Deters or her staff at the time of your appointment.

Court Ordered for an Evaluation

If the patent(s) are referred for a court-ordered evaluation/assessment…

Checklist

Patients Without Insurance

If an adult or child will not be using insurance to cover the cost of treatment follow these guidelines:

  1. Complete the intake packet prior to your appointment. Appointments do not allow for time to complete paperwork; hence, if you wait until then to complete the intake packet, you will spend less time discussing your concerns and may need another appointment for such. The forms, office policies and other documentation provided to you have been suggested by your insurance company, the American Psychological Association and the Board of Psychology/state regulations. The volume of documentation is large, so reserve time before your appointment to complete it.
  2. Arrive with payment which will be accepted in the form of check, credit card or cash.
  3. Please remain in the waiting area and you will be greeted by Dr. Deters or her staff at the time of your appointment.

Couples or Families with Insurance

If two or more individuals (such as a couple) are seeking couples, co-parenting, or family counseling and wish to use insurance here is a guideline:

  1. Ask your insurance carrier to determine if Dr. Jean Deters is in EACH of your network and if your policy covers outpatient treatment in an office setting. Dr. Deters will not be able to submit your claim if your benefits do not use her as a provider; hence, you will be expected to pay the full charge of the session at the time of service unless she is in your network.
  2. Ask your insurance carrier what your deductible is, if it has been met as well as what your co-payment /co-percentage is for the codes 90791, 90834, 96101, 90847, and 90846. A deductible is the amount you must pay out of pocket before you can begin paying only a co-payment/co-percentage. A co-payment or co-percentage payment is a smaller amount of the cost of service for which you are responsible, after your deductible has been met. For example, you may have a $3000.00 deductible which must be paid prior to using a $20.00 co-payment. If Dr. Deters is in your insurance network and you have not met your deductible, your out of pocket expense will be lower than her full fee due to contract agreements with your insurance company.
  3. Complete the intake packet prior to your appointment.   7. After Dr. Deters’ administrative assistant reviews insurance policies, you will be informed of the best insurance options available to you.
  8. One patent will be identified for billing purposes. The other participants will complete a NON PATIENT participation agreement.

Children with Insurance

If a child patient is being referred for general psychological services and wishes to use insurance follow the following guideline:

If the child patient’s parents are NOT married:

  • When a parent has been awarded SOLE custody (which is not the same as primary or residential status in a joint custody arrangement), then he/she has been granted authority by the Court to make medical decisions for the child(ren), even to the exclusion of the other parent. If a parent has SOLE custody, a copy of the Court’s order is required to proceed with consent for treatment with Dr. Deters
  • When parents have been awarded JOINT custody (again, regardless of primary or residential status), then both have been granted authority by the court to make medical decisions for the child(ren). In the case of joint custody, consent from both parents is required to proceed with Dr. Deters.
  • Separated, but not divorced, parents will be asked to co-sign consent.

If the child patient’s parents are married:

  • When parents are legally married, only one parent must sign consent for the child to be treated by Dr. Deters.

  7. Dr. Deters typically meets with parents alone (without the child) for the first appointment. In cases of two household families, Dr. Deters may also meet with each parent separately after the initial combined parent interview.

Individual Adults with Insurance
